We are coming together to learn from each other, share best practices, and explore innovative approaches that make a difference in the lives of children, adults, and families.

We can only fix them if we use comprehensive community problem-solving efforts rather than single-focused approaches. This is developing in 84 Indiana cities in 37 counties.

In this session we will introduce you to the first four steps of Collective Community Impact: Step #1 — Prepare Yourself for the Journey; Step #2 — Invite Others to Travel with You; Step#3 — Set Up Your Basecamp; Step #4 — Experience the Value of Working Together.
Each step will introduce you to the Key Challenges, Action Steps and Measurable Results that will help you identify where you are along the journey as well as opportunities for engaging the community in action. We are fortunate to also have our Partners In Practice that will discuss their community’s journey toward Collective Community Impact and how they are utilizing Charity Tracker to assist their work and achieve their goals.
We are taking a break from learning about Collective Community Impact to focus on a more foundational topic, but one that you may not be familiar with: ACES.
ACES is acronym for Adverse Childhood Experiences and learning about ACES is part of Trauma-Informed Care, which “is a strengths-based framework that is grounded in an understanding of and responsiveness to the impact of trauma and that emphasizes physical, psychological and emotional safety for both provider and survivors, and that create opportunities for survivor to rebuild a sense of control and empowerment.” (Hopper, Bassuk, & Olivet, 2010)
Our Speaker for this lunchtime session is Tonya Carter of C&C Empowerment Group and the Carter Education & Community Foundation. Tonya has over 25 years of experience working with individuals and families, and she is dedicated to helping people make transformational changes whether in their personal or professional life.
She is a is a trained Community Health professional, Program Management Specialist and Empowerment Advisor. She is also the Founder and Executive Director of Unique Family Connections, a nonprofit dedicated to building healthier and empowered communities.
We will introduce you to the last four steps of Collective Community Impact: Step #5 — Set Up More Basecamps Along the Way; Step #6 — Bridge the Gaps That Advance Forward Progress; Step #7 — Map Out a Successful Path; Step #8 — Create Wide Pathways for Many Explorers to Travel. We’ll continue to dive into the Key Challenges, Action Steps and Measurable Results that will help you identify where you are along the journey as well as opportunities for engaging the community in action.

Now we’re ready to get more technical. Around the country and across Indiana, Network Administrators are working to bring their community’s Helping Agencies together for the purpose of Distributed Transformative Network and that happens through CharityTracker.
We will explore how CharityTracker is used for Case Management across a Network of Providers, in an effort to provide Comprehensive Coordinated Care for those in need. This includes the following topics: Common Intake for Shared Clients, Case Management with Appointments and Outcomes, Comprehensive Referral Network Managing Clients Outside the Office using Kiosk Mode, Reporting and Data Analytics. This session will teach you all of the practical things you need to know about how to use CharityTracker to achieve your goals.
We will discuss how to Host a Community Conversation using the Harwood Model and Angie Williams from SPARK Consulting will walk us through the best practices and share her experience hosting Community Conversations.
